A well documented phenomenon that occurs
to young people who are placed out of their homes for more than the briefest period is
the complete disruption and discontinuity of services that might otherwise lend critical structure and normality to their lives.
Lighthouse Youth Services has developed a sophisticated complex of services to ensure continuity and transition of service for young people who are in custody of the child welfare system - Hamilton County Department of Job and Families Services, Hamilton Choices or
any other public custodian.
In this system, young people may transition through Lighthouse therapeutic group homes, foster care, independent living and back to community placement with families or relatives – all the while maintaining a continuous primary therapist, stable pharmacological management, a single special needs school environment and in some cases, a single oversight program – Gender Responsive Therapy for Girls or Community Management Services for high risk youth.
In addition to these structured transition services, the Home Based Services Division of Lighthouse can adapt and mold services to an extraordinary degree in order to facilitate successful program step downs, transitions and discharges to the community.
We encourage public service case managers to explore safe and successful transitions for the young people whom they serve.
